The Sierra Health Foundation

The Sierra Health Foundation (SHF) is a private philanthropy that forges new paths to promote health and racial equity in partnership with communities, organizations and leaders. SHF is committed to reducing health disparities through convening, educating and strategic grantmaking. Sierra Health Foundation’s funding region includes 26 counties in Northern California. In 2015 SHF launched The Center at Sierra Health Foundation to support communities statewide in accessing and leveraging resources to create healthier communities for all.

The Center at Sierra Health Foundation, a 501© (3) nonprofit organization created in 2015 and controlled by Sierra Health Foundation to bring people, ideas and infrastructure together to help communities across California to access proven practices for creating healthier communities, while tapping into their existing resources, knowledge and creativity.  With offices in Sacramento and Fresno, The Center was developed at a time when nonprofits and public agencies throughout California faced diminishing resources and rapidly increasing needs.

San Joaquin Valley Health Fund

The San Joaquin Valley Health Fund (SJVHF) strengthens the capacity of communities and organizations in the San Joaquin Valley to improve health and well-being by advancing programs and policy changes that promote community health and health equity for all. Managed by the Center at Sierra Health Foundation, SJVHF serves as a conduit for grant support from the Sierra Health Foundation, Blue Shield of California Foundation, California Wellness Foundation, Dignity Health, Hellman Foundation, James Irvine Foundation, Rosenberg Foundation, The California Endowment, Tides, W.K. Kellogg Foundation, and Wallace H. Coulter Foundation and Convergence Partnership.
